Student Acknowledgment/Agreement for Audio Recording Lectures
The Agreement for Audio Recording is required if a student has been approved for the academic accommodation of audio recording. This form must be reviewed, initialed and signed by the student prior to the Faculty Notification of Accommodation being provided. The Release of Information allows the student to give Student Access & Accommodation permission to discuss and release relevant information to the approved offices/and or individuals. The form must be reviewed, initialed and signed prior to the faculty notification of accommodation being provided.
